Learning-Oriented Failure Reviews and Innovation Continuity

Abstract Organizations pursuing innovation inevitably encounter unsuccessful experiments, implementation errors, abandoned concepts, missed assumptions, prototype defects, delayed projects, and market responses that diverge from expectations. Whether these events weaken subsequent innovation or become productive learning inputs depends substantially on how organizations interpret and review failure. This study develops the concept of learning-oriented failure reviews as structured post-event processes designed to identify what occurred, reconstruct causal mechanisms, distinguish preventable error from legitimate experimentation, preserve useful knowledge, and convert lessons into subsequent experimentation. The study investigates the relationship between failure-review maturity and innovation continuity, defined as an organization's capacity to sustain experimentation, knowledge development, and innovation activity after unsuccessful outcomes without repeating avoidable mistakes or suppressing responsible risk taking. Established research indicates that failure can create unusually valuable learning opportunities, but learning does not occur automatically. Psychological safety, accurate analysis, motivation, organizational attention, and the capacity to transfer lessons influence whether failure information becomes usable knowledge.
Because no primary organizational dataset was supplied, the present study adopts a simulation-based explanatory design. Five synthetic scenarios representing failure-review maturity from 20% to 100% were evaluated through indices of psychological safety, causal-analysis quality, knowledge retention, corrective experimentation, blame orientation, and innovation continuity. The simulated Innovation Continuity Index increases from 54 to 90 as review maturity increases, while blame orientation declines from 76 to 31. The numerical findings are methodological illustrations rather than empirical estimates. The study concludes that failure reviews support innovation most effectively when they preserve accountability while avoiding indiscriminate blame, differentiate intelligent experimentation from negligent error, and translate lessons into concrete changes in future projects.
