Heat-Health Preparedness Among Informal Urban Workers

Abstract Informal urban workers frequently undertake physically demanding work in streets, markets, construction zones, transport corridors, waste-management sites, small workshops, and other environments where exposure to high ambient temperature may be difficult to avoid. Their vulnerability to occupational heat cannot be understood exclusively through environmental exposure because protective capacity is also shaped by access to drinking water, shaded recovery space, sanitation, work-rest opportunities, schedule flexibility, heat-risk information, healthcare access, and the economic consequences of stopping work. Indian occupational studies have documented substantial heat exposure, adverse health symptoms, and reduced work capacity across organized and unorganized work settings, while systematic evidence demonstrates that occupational heat strain adversely affects worker health and productivity.
The present study develops a simulation-based analytical framework for examining heat-health preparedness among informal urban workers. Preparedness is conceptualized as the practical capacity to recognize dangerous heat, receive and understand warnings, remain adequately hydrated, access shade and recovery opportunities, modify exposure where feasible, identify escalating symptoms, and obtain appropriate medical assistance. Because no original worker dataset was supplied, 600 synthetic worker profiles were generated for methodological illustration. Numerical results are explicitly simulated and should not be interpreted as observed prevalence or causal estimates.
The simulated analysis identifies a strong inverse relationship between heat-health preparedness and heat-related functional disruption, with a modeled correlation of r = −0.70. Mean functional-disruption scores decline from 61.6 in the lowest-preparedness group to 34.2 in the very-high-preparedness group. The model further indicates that preparedness cannot be reduced to worker knowledge alone. Access to water and shade, control over working hours, and the economic feasibility of taking protective action substantially influence whether awareness can be translated into effective risk reduction.
The study proposes a three-level preparedness model integrating worker capability, work-environment protection, and city-level heat governance. Heat-health action planning, occupational adaptation, public warnings, healthcare readiness, water and cooling infrastructure, and income-sensitive work arrangements should be viewed as complementary rather than competing strategies. Ahmedabad's heat-action experience demonstrated that coordinated heat warnings, public-health communication, and healthcare preparedness can be operationalized at city level, while subsequent evaluation reported lower summertime mortality after implementation, especially at extreme temperatures.
The paper concludes that informal workers should not be classified as adequately prepared merely because they understand heat risk. Effective preparedness exists when workers also possess the material, occupational, social, and institutional capacity to act on that knowledge without experiencing disproportionate economic loss. The framework offers a basis for future field studies combining environmental heat measurement, occupational surveys, symptom monitoring, work-capacity assessment, and geospatial mapping of protective urban infrastructure.
