Public Understanding of Risk in Direct-to-Consumer Genetic Testing

| Abstract | Direct-to-consumer genetic testing has expanded public access to information concerning genetic ancestry, carrier status, selected health predispositions, traits, and susceptibility to complex diseases. The accessibility of such testing changes the conventional pathway of genetic information because consumers may encounter probabilistic health-risk estimates without receiving pre-test or post-test interpretation from a genetics professional. This raises an important public-health question: whether consumers understand that a reported genetic predisposition represents one component of disease risk rather than a deterministic prediction of future illness. Earlier research demonstrated that members of the general public can perceive direct-to-consumer genetic reports as easy to understand while nevertheless interpreting their meaning incorrectly. Leighton, Valverde, and Bernhardt found significant differences between public and genetic-counselor interpretations of sample genetic-risk reports, particularly concerning the clinical meaning and usefulness of reported risks. The present study develops a simulation-based framework for examining public understanding of genetic risk. Because no original participant dataset was supplied, 600 synthetic consumer profiles were modeled across three interpretation conditions: report-only disclosure, contextualized plain-language explanation, and genetics-professional-supported interpretation. Risk comprehension was conceptualized through understanding of probability, recognition of multifactorial disease causation, distinction between relative and overall risk, interpretation of negative results, and recognition of the need for confirmatory clinical testing when appropriate. Numerical findings are explicitly simulated and must not be treated as population estimates. The study argues that public understanding of direct-to-consumer genetic testing should not be measured simply by asking whether reports appear clear or useful. Effective risk literacy requires the ability to recognize uncertainty, integrate family history and environmental influences, understand that tested variants may represent only part of relevant genetic variation, and resist both fatalistic interpretation of increased-risk findings and inappropriate reassurance from lower-risk findings. The paper concludes that direct-to-consumer genetic-risk communication should combine transparent limitations, absolute-risk context where scientifically justified, visual and plain-language explanations, access to qualified professional interpretation, and explicit guidance against making major medical decisions from unconfirmed consumer-generated results. |
